Our RN-licensed Community Health Director and team of Certified Nursing Assistants provide professional and compassionate personal care and health supportive services in the privacy and comfort of your own apartment home. Our team will work with you to develop a holistic, individualized care and wellness plan. Additionally, our Community Health Director will assist you to manage and monitor chronic healthcare conditions, communicate with your healthcare providers and spouse or family, coordinate healthcare appointments and transportation, and help you to manage your medication regimen and pharmacy services.

We offer an extensive range of Assisted Living Services, including:

• Bathing, dressing, grooming and continence support
• Mobility, transfer, and escort assistance
• Medication management and assistance
• Dietary and nutritional support
• Social support for life enrichment programs and events
• Memory support

Assisted Living and nursing care services are offered on an a là carte basis so that you only pay for the services if and when you need them.
